43 patients treated with thrombectomy:
22 with pigtail + 6F MPA catheter

10 with AcoStream
Small-dose urokinase used in some cases based on bleeding risk.
✅ Success & Mortality
95.3% success rate (41/43 patients)
2 deaths (1 due to pulmonary infection, 1 due to intracerebral hemorrhage at 9 months).
1 recurrence of pulmonary embolism.
5 patients had persistent chest tightness.
✅ Clinical Improvements (P<0.001)
Better cardiac function (cardiac index, mean arterial pressure, oxygen saturation).
Reduced pulmonary artery pressure & right atrial diameter (echocardiography).
Stable blood markers (except hemoglobin, lactate, and oxygen levels, which changed significantly).
✅ Complications
9 cases of transient cardiac arrest.
7 cases of local hematoma.
3 cases of puncture site infection.
✅ Follow-Up
41 patients followed up, with 1 late death and 1 recurrence.
AcoStream and other thrombectomy methods showed high success rates and clear post-op improvements, with manageable risks.
Conclusions: PMT can be used as a safe and effective treatment option for acute medium-to-high-risk pulmonary embolism.
